4Life Home Health Care opened in 2024 and is administered by Registered Nurses with extensive experience serving the EEOICPA community. Our company was built on the belief that every patient deserves compassionate caregivers, skilled nursing oversight, and personalized attention tailored to their unique needs.

As experienced healthcare professionals serving patients under the EEOICPA program, we recognized the need for a more patient-centered approach to home health care. While many agencies focus heavily on marketing and patient incentives, we believe the primary focus should always be exceptional clinical care, patient advocacy, and meaningful support for the individuals and families we serve.

At 4Life Home Health Care, our mission is to provide compassionate, high-quality home health care services to individuals covered under the Energy Employees Occupational Illness Compensation Program Act (EEOICPA). Care Team Roles

Our Team & Leadership

Administrator: Administer, direct, and coordinate all administrative and financial activities of the Home Health Agency.

Assistant Administrator/ Director of Nursing: Act as Administrator in the absence of the Administrator and directs and coordinates all administrative and financial activities of the professional Home Health Agency. Assists with the planning, coordination, leading, controlling, and evaluation of home health services.

Administrative Assistant: Assists the Administrator, Assistant Administrator, and Director of Nursing. Coordinates all onboarding paperwork for new caregivers. Manages annual compliance requirements for all employees and contractors. PCA Trainings.

Nurse Managers: Registered Nurses who provide support, management, and oversight of care team. Includes Care Coordinators, and in home licensed (CNA, LPN, RN) and unlicensed (HHA/PCA).

Care Coordinators: Responsible for the coordination of patient care. Attend physician appointments with you regarding home health care. Coordinate additional testing, equipment, and services. Weekly communication with patient/family and care team.

Patient Account Specialist Manager: Supervises billing staff. Reviews all licensed (LPN/RN) documentation for accuracy. Contacts caregiver for corrections on documentation. Prepares documents for claims submission and billing.

Patient Account Specialist: Reviews all CNA/PCA/HHA documentation for accuracy. Contacts caregiver for corrections on documentation Assist Pt Account Specialist Manager with billing claims.
